1.) Membership Status Verification Slip (MSVS- applicable for Pag-ibig Housing Loans ) original & 2 photocopies.
2.) Certification of Employment & Compensation issued by th employer under oath (original & photocopy).
3.) BIRTH / MARRIAGE CERTIFICATE (photocopy).
4.) Latest Income Tax Return (ITR) with W-2.
5.) Latest Payroll / Pay Slip (certified copy).
6.) 3 copies 1x1 pictures with signature @ the back (husband & wife if applicable).
7.) TIN Card, SSS ID, EMPLOYMENT ID (photocopy)
8.) Resident Community Tax Certificate (CEDULA).
9.) Proof of Billing (Lights or Water Bill).
10.) Special Power of Attorney (SPA) - HDMF (Pag-ibig format).
11.) Valid JOB CONTRACT for OFW.
12.) PASSPORT & VISA for OFW (photocopy) (for OFW).
13.) Audited Financial Statement with CPA report, Buiness Permit, DTI Certificate, ITR with Receipts for Self-Employed.
14.) Post Dated Checks (PDC) for the monthly amortization (for 2 years) and equity payments.
